Eligibility :
Passed Intermediate, 10+2 or equivalent examination in English, Physics,
Chemistry and Biology (Zoology + Botany) taken together;
(i) For MBBS/BDS/BAMS/B.Pharm.(Ay.) - A minimum of 50% for general and
40% aggregate marks for OBC/SC/ST candidates;
(ii) For B.Pharm. - A minimum of 60% marks for general & OBC and
55% aggregate marks for SC/ST candidates. Candidates appearing in qualifying
examination may also apply.
Distribution of Seats:
M.B.B.S.(84 seats) : General - 42; OBC - 23; SC - 13 and ST - 6;
BDS and B.Pharm. (38 seats each): General - 19; OBC - 10; SC - 12and
ST - 6;
BAMS (78 seats) : General - 36; Foreign National - 3; OBC - 21; SC -
12 and ST - 6;
B.Pharm.(33): General - 17; OBC - 9; SC - 5 and ST - 2.
Three percent seats are reserved for Physically challenged candidates
in all the categories of the above courses.
Age : Completed 17 years but not crossed 25 years
as on 31st December, 2012 (Candidates born after 1st January, 1996 and
on or before 31st December, 1987 need not apply).

By Postal Channel : From the Office of the Director
(PMT-Cell), Institute of Medical Sciences, Banaras Hindu University,
Varanasi-221 005 by sending a written request through Registered Post
along with Crossed Demand Draft/Bankers Cheque (MICR only) of Rs.
1600/- (Rs.1100/- for SC/ST candidates) of a Nationalized Bank, drawn in favour
of "The Director, Institute of Medical Sciences, BHU" payable
at Varanasi. Application form will be sent by Speed / Registered Post.
However, Institute will not be responsible for any loss or postal delay.
The request for application form must include the name and address of
the candidate, and full particulars of the enclosed DD/BC (write candidate's
name and address on the back of DD). Two self-addressed, preferably
typed, white paper slips of 10 cm 6 cm size MUST also accompany it.
The envelop containing requisition for PMT application form should
be super scribed "Request for Application for PMT-2012, BHU".
Incomplete/non-cashable Draft (under amount/unsigned/ without Bank
Code No. & Seal) shall not be accepted.
Note :
(a) Separate Bank Draft will be required for each application form.
(b) MO/I.PO/Cheques shall NOT be accepted under any circumstances.
Application Forms from the Office of the Director (PMT-Cell), Institute
of Medical Sciences, can be obtained by POST only and not in person.
In case forms are not available at Indian Post Offices, the same can
be obtained by post from Director's Office upto 27 February, 2012.
Application form duly completed must be sent through Registered Post
so as to reach THE DIRECTOR (PMT CELL), INSTITUTE OF MEDICAL SCIENCES,
BANARAS HINDU UNIVERSITY, VARANASI-221 005. on or before the prescribed
date.
